Future plans for Hope’s Home include this service. Transition care is a temporary, pre-planned admission to the centre for a medically fragile child who is medically stable enough to be discharged from an acute care facility, but whose community requires more training to care for the child.
Part of Hope’s Home future plans include long term care for medically fragile children. This care will be an alternative to living in a hospital, and will provide a medically safe environment within a family-centered and inclusive home environment.
In the immediate future Hope’s Home seeks to expand its evening/weekend respite
services to include temporary 24 hour/overnight respite care for families of medically fragile children.
